Patients enrolled in the NextGen patient portal (the system used by Dr. Barrett before he retired) may download a summary of their medical record through that portal. For most patients this will provide their new doctor with all the information they need.
To request a more complete copy of your records, you can fax a request to Dr. Barrett at 888-296-3635 or mail a request to Bart Barrett, MD c/o Edinger Medical Group, 9900 Talbert Avenue Suite 301, Fountain Valley, CA 92708.
Your request must include patient's printed name and original signature of the patient, (or parent if patient is a minor), and include:
